Fill out test request form fully & legibly. Clinical history, CBC, PBF report findings must be provided. Sample type must be specified, I.E. Peripheral blood vs. Bone marrow. The specific panel required (Acute Leukemia vs. Chronic Leukemia/ Lymphoma) must be specified based on outcome of baseline investigations.

EDTA whole blood/ bone marrow plus Heparin whole blood/ bone marrow plus well prepared, labeled & fixed slides.

Container/ preservative

EDTA (PURPLE TOP) & HEPARIN (GREEN TOP) vacutainers. slides should be prepared from the EDTA vacutainer.
